SWAN Hill District Health is bridging gaps in health care, with soon-to-be eight qualified nurse practitioners.

The health service will celebrate their work during Nurse Practitioners Week, from December 11 to 17.

Nurse practitioners, a cornerstone of modern health-care, are registered nurses empowered to practice independently and collaboratively in an expanded clinical role.

The distinctive features of nurse practitioners make them instrumental in shaping health-care outcomes.

They practice in all areas and sectors of health care, providing crucial services to populations deemed vulnerable in metropolitan, regional, rural and remote settings.

Patient-focused and person-centred, nurse practitioners can diagnose, treat, prescribe medications, order tests and initiate referrals, ensuring a comprehensive approach to health care.

SHDH’s nurse-practitioner team is a shining example of excellence.

Specialising in emergency and critical care, aged care, chronic and complex disease management, mental health, sexual health, AOD and more, these professionals are dedicated to meeting the unique health-care needs of the Swan Hill community.

The addition of Alex, Chloe, Di and Hollie, brings the total number of nurse practitioners and nurse-practitioner candidates to an impressive seven by the end of the year.

Cass, who has nearly finished her degree, is now practicing in an advanced level and will soon be a great addition to the team.


Closing gaps in regional health-care

NURSE practitioners at SHDH are not just health-care providers – they are advocates for the community.

Recognising the gaps in regional health-care, these professionals work collaboratively to ensure continuity of care, especially in areas where general practitioners may be scarce.

Their ability to work autonomously and independently, coupled with strong links to multidisciplinary teams, positions them as leaders in case management and care co-ordination.


A salute to Swan Hill’s health-care heroes

SHDH nurse practitioner Trish Oxley said the team was “thrilled” to be part of the week-long celebration.

“It provides a fantastic opportunity to showcase the vital contributions of nurse practitioners to health care,” she said.

“Nurse practitioners are a cornerstone of the health-care system, and we are proud to support and celebrate their outstanding work.”

The community has been encouraged to join in the celebrations to “engage in meaningful discussions, share educational resources and celebrate the incredible work of nurse practitioners”.
