SHDS supports residents to stay at home

AT Swan Hill District Health, the team appreciates how special “home” is to you and wants you to enjoy staying at home for as long as possible.

“We know that some things can be hard for you to do alone, but we are here to help,” a SHDH spokesperson said.

“Our healthcare workers are able to assist you with personal care including help with showering, dressing, mobility and meal preparation.

“Our team also offer a flexible respite service allowing carers to have a break knowing that the person they care for is being looked after and supported within their home environment.”

The Commonwealth Home Support Program team can provide the information that’s need to help people to access in-home services.

“They are able to help you access My Aged Care for advice and assessment processing and they will then link you with one of our healthcare workers to meet and discuss your needs and preferences and create a personalised plan for you and your carer,” the spokesperson said.

“Give us a call today on 5033 9229 Monday to Friday 8.30am-4.30pm.

“We know that accepting this help can sometimes be a tough decision so allow us to introduce you to the staff: our dedicated team of healthcare workers ready to attend your home to provide these services.”
