Achieving and maintaining optimal independence

THE Community Rehabilitation Centre provides short-term, goal-oriented rehabilitation services to members of the Swan Hill community and surrounds.

The services on offer are aimed at individuals who have a disability, have a chronic health condition or are recovering from injury, medical or surgical conditions.

The CRC aims to assist individuals in achieving and maintaining their optimal independence in their chosen activities.

Services available

A range of outpatient centre-based and home-based allied health/nursing appointments are offered through the centre as well as specialist clinics.

Specialist clinics offered through CRC include:

     persistent pain program

     continence clinic

     falls and mobility clinic.

Group programs include:

• orthopaedic group

• cardiac rehabilitation

• osteoarthritis group

• heart failure program

• falls and balance group

• pulmonary rehabilitation/asthma education

• neurological group

• supervised exercise group (gym time)

• tai chi for arthritis

• Nordic walking group.

The multidisciplinary team includes physiotherapy, occupational therapy, speech pathology, exercise physiology, podiatry, dietitian, continence nurse, respiratory nurse, cardiac nurse, pelvic floor physiotherapy, psychology, specialist medical practitioners and allied health assistants.
