Learn more, feel better with HARP

DO you or anyone you know have a chronic disease?

These may include: diabetes, lung conditions (e.g. COPD/Asthma/emphysema), heart conditions (e.g. chronic heart disease), complex health care needs.

You may be eligible for our no cost Hospital Admission Risk Program (HARP). HARP is a community support program based at Swan Hill District Health. This service aims to improve the health experience and quality of life for people who are living with chronic and/or complex health issues.

Care is provided by care coordinators who have a nursing or allied health qualification. Your allocated care coordinator will undertake a thorough assessment of your needs and will work with you to create a plan of care that clearly describes the services that would benefit you and understand the goals you would like to achieve.

HARP can help by: providing education to help you manage your condition and/or medications, coordinating appointments with specialists, GP’s and other health professionals, providing emotional support, linking you with support and community health services, assist in preventing unplanned hospital admissions
