VICTORIA’S healthcare quality and safety improvement agency has put Swan Hill District Health (SHDH) under the microscope, examining its systems and response to clinical deterioration.
Safer Care Victoria (SCV) visited SHDH in December 2017 to meet with staff and tour facilities, and has provided training and required resources in line with the coroner’s recommendations.
The coroner’s recommendations related to the death of 36-year-old Lynsie Everett from Woorinen, who died from complications of pneumonia at the health service in 2015.
The case of Ms Everett was aired on national television last Thursday on 7.30.
The Guardian understands SCV returned to the health service last week to meet with board members, executive staff and community members and discuss ways to implement positive change and re-assure the community of the quality and safety of their local service.
The agency’s chief executive officer Euan Wallace said SHDH had undertaken a “significant amount of work” over the past two years to address issues since the death of Ms Everett.
